Payment & Insurance: A credit card is required to hold your request.
- Insurance Patients: Your card will only be charged on the day of your appointment for copayments, deductibles, or coinsurance.
- Self-Pay: The full fee will be processed on the day of your visit.

Immediately after booking, you will receive intake forms via email.
Critical Note: Please complete these forms within 48 hours.
Dr. Lee’s Personal Review
Your appointment is tentatively held as soon as you book! Dr. Lee will do a quick review of your forms just to make sure she’s the best fit for your specific needs so you get the highest quality care from minute one.
Final Confirmation
Once paperwork is approved, you will receive a final confirmation. If forms are not done within 48 hours, the time will be released.
